Restorative dentistry

The field of stomatology dealing with caries and pulp treatment is called restorative dentistry. The filling is very aesthetic and long-lasting owing to the use of most modern methods and materials. The procedure itself is absolutely painless.

Application of microhybrid (FILTEK Z250) and nanocomposite (FILTEK SUPREME XT) 3MESPE materials provides obtaining the colour of the filling which exactly matches the colour of the patient's tooth. The gloss and the match remain perfect even after five years after the filling has been applied.

In many cases these materials are used to rebuild the tooth which has been qualified, by other dentists, to prosthetic reconstruction with a crown (a crown is a much more expensive solution).

Esthetic dentistry - teeh whitening

For many of us, having a complete set of healthy teeth has become insufficient. We would like to be more attractive and we are becoming more and more demanding towards ourselves and towards others. A beautiful smile can make us feel younger and more attractive.

A recent research shows that the teeth is the second part of a face (eyes is the first) that a person we are talking to focuses one's eyes on.

The whitening process can be applied to whole arches of teeth or just to a single darker tooth (e.g. a tooth after a root canal therapy). There is a variety of whitening methods. However, the safest method is always to perform the whitening operation under the supervision of a dentist.

The BEYOND whitening method
The whitening agent used in this method is based on hydrogen peroxide and silicon dioxide, and it is activated by light from a special lamp (the Beyond Whitening Accelerator).

Before the operation
- the teeth should be healthy
- the gums should be healthy
- tartar is removed

During operation
The teeth are covered with the whitening agent which is subject to light emitted by the Beyond lamp. The light activates and accelerates the whitening process. The whole operation lasts for approximately forty-five minutes.

Safety
Long-term testing and clinical research have shown that teeth whitening is completely safe for the teeth, gums as well as bridges, crowns or fillings. Enamel structure is not weakened and caries susceptibility remains at the same level.

The effect
The whitening effect is long lasting. It's worth remembering, that teeth naturally darken over years. Smoking cigarettes, drinking tea, coffee, wine, dark juices, etc. accelerates the process. The whitening process can be repeated after a few years period, if necessary.

After the operation
Within 48 hours after the operation the patient should be on a "white diet" (one should intake diary products in order to preserve the whitening effect). One should also use a toothpaste that does not contain fluorine. During the next 10 days strong tea, black coffee, black currant juice or red wine should be avoided.

Limitations:
allergy to hydrogen or carbamide peroxides, very seldom

Other limitations are temporary in default of research:
- application during pregnancy or lactation
- application in children under 16 years old

Another method used in our clinic is overlay night whitening.
In a prosthetic laboratory overlays are made individually for every patient. The patient gets the overlays along with whitening gel which are applied to the teeth every day. The gel operates on the teeth during the contact period (all night). The procedure is repeated from a few days to 3 weeks depending on the requirement. The effect is also long-lasting and with proper mouth care, lasts for a few years.

Prosthetics

Implantology

What are implants ?
Implants are screws made of titanium which, when planted in the bone, replace the human root of a tooth. The implant forms union with the bone thanks to the properties of titanium - osseointegration takes place (osseus tissue is formed around the implant and fibrous tissue does not grow in between the implant and the bone).Then, using special connecting bars, prosthetic appliances (crowns, bridges, dentures) are mounted onto the implants. Implants can be used to reconstruct:
- single or several teeth
- all teeth

Professor Brenemark from Sweden is the creator of the tooth implant and his implants have been made by Nobel Biocare corporation. Our clinic is an authorised partner of Nobel Biocare.

Implantation procedure:
During the implantation the patient is under a local anaestheic. An inlet of a respective diameter is dissected in the bone, next the implant is put in the inlet. The procedure is absolutely painless and can be compared to a simple extraction of a tooth.

Prothetic appliances:
The patient must wait 3-6 months from the implantation procedure (the implant has to merge with the bone. Only then prosthetic work can be started.
At this stage the patient has a temporary appliance which is both aesthetic and functional.
Then, a permanent appliance is made which takes approximately 1-2 weeks depending on the type of appliance.
Sometimes it is possible to mount prosthetic appliances on the implantation day, however, a doctor makes the decision after a series of examinations.

Why to undergo implantation procedures?

- implants provide comfort and stability for crowns and partial dentures

- serious systemic illnesses
- using immunity-lowering drugs
- insulin dependent diabetes
- mental illnesses

Warranty:
Nobel Biocare provides patients with 10 year warranty which guarantees that in case of loss of an implant te company will replace the rejected implant free of charge. The patient is only subject to re-implantation procedure.

The dentist makes a first examination and takes one or more x-rays of the area to prepare for the procedure.

The implant is installed. At this time, a temporary tooth is provided that allows you eat and function like normal almost immediately.  The implant will need a few months to integrate with the jawbone before the next step is taken.

The final step is the placement of the permanent ceramic tooth. The new tooth is installed for life. No additional treatment is needed.

You should expect the new tooth to fit and function just like a natural tooth. Do your usual dental hygiene to keep the tooth and gum around it clean and healthy.

Other alternatives:

Tooth-supported fixed bridge
A traditional bridge involves grinding down adjacent teeth to support the bridge. It is a stable solution with good esthetics and function that is fairly easy to install. However, this alternative has two main disadvantages: continuous bone resorbtion in the edentulous area, and sacrificing healthy teeth on behalf of the bridge.
Removable partial dentureRemovable partial denture
This is not a permanent alternative to a lost tooth. It is unstable and loosely attached, which affects both function and comfort. A removable partial denture is made of plastic – a material that can't create the same esthetic result as a ceramic crown. The benefits are few but do exist: adjacent teeth aren't affected. It is easily and quickly installed and relatively cheap.

This alternative has some clear advantages: it is quickly installed, functions well and, since it is made of ceramic, it gives a high esthetic result. Moreover, natural healthy teeth aren't affected. But it is not very permanent. The resin-bonded bridge will eventually come off – probably after just a couple of years – and will then have to be reinstalled.
